Preparing for your visit
Before your visit, we ask that you print, fill out, and bring the following:
Individual Forms:
Please also bring your current insurance card, any required copays and a referral from your primary care physician, if required, to every visit. Unfortunately, we will not be able to see a patient for their appointment without these due to insurance rules.
