Survivorship 

An oncology survivorship care plan is a comprehensive, personalized roadmap designed to help cancer survivors navigate life after treatment. It includes detailed information about the patient's cancer diagnosis, the treatments received, potential long-term side effects, follow-up care schedules, and strategies for maintaining health and well-being. This plan ensures that survivors have a clear understanding of their health needs and a structured approach to post-treatment care.

Our nurse practitioners at St. Louis Cancer Care specialize in creating tailored survivorship care plans for our patients once they have completed their therapy. These plans are customized to address each individual's unique experiences and future health considerations, providing support and guidance for the next phase of their journey.

Resources for Survivors 

We offer a list of online resources about survivorship to help our patients stay informed and connected. These resources cover various aspects of life after cancer, including physical health, emotional well-being, and community support, ensuring that our patients have access to the information and assistance they need to thrive.

Cancer Support Community (CSC) 

Cancer Support Community (CSC) offers a helpline, online support (“The Living Room”), support groups, and educational resources.  CSC maintains an affiliate network offering free in-person services including counseling, wellness programs, and social support

Centers for Disease Control (CDC)

The CDC gives guidance on survivorship care plans including the recommendations for follow ups, testing, long term side effects, and wellness recommendations 

Imerman Angels 

Imerman Angels facilitates one-on-one peer mentoring by matching survivors or caregivers with a trained survivor “mentor angel.” Services are free and personalized

Livestrong

The Livestrong Foundation provides resources and support for people affected by cancer, focusing on improving quality of life during and after treatment. Through partnerships and community programs, Livestrong helps survivors and families navigate the physical, emotional, and practical challenges of cancer survivorship
